About the Author
NANCY WAINER COHEN co-founded C/SEC, Inc., the first and now the largest cesarean organization in the country, in 1973 and is a consultant and advisor to the Cesarean Prevention Movement. She is a leading authority on VBAC and has counseled thousands of women on the subject since 1972. Her many articles have dealt with VBAC, cesarean prevention, and positive birthing. Her personal birthing history includes a cesarean delivery for her first child, a vaginal birth in the hospital for her second child, and a vaginal birth at home for her third.
LOIS J. ESTNER, formerly an English teacher, is a VBAC mother, La Leche League leader, and counselor on the topics of breastfeeding, childbirth, and cesarean prevention.

Every pregnant woman should read this book

Wow...this may be THE birth book every woman pregnant needs to read. Some sections are specific to those of us trying to have a vaginal birth after cesarean (VBAC), but much of the book is aimed at preparation for any birth. It's most important contribution may be teaching all women how to prevent a c-section in the first place unless truly unnecessary (most of them, sadly ,aren't). As a VBAC mama, I especially needed to hear the chapter on Uterine Dependability - so helpful with the scare tactics used by most doctors about uterine rupture.

Nancy Cohen (main author) tells us like it is--that c-section birth rate is overly high and that the interventions are too demanding at hospitals. The only real way to have a natural birth with no interventions is to have a home birth because the hospitals will want to hook you up to an IV and monitor the baby with outside monitors on the belly (at least that) and not feed you because of hospital restriction rules while in labor. The "business of being born" makes you feel like a chewed up piece of gum that got spat out when you are through having your baby and oh, here's the bill too! (If you are not on Medicaid--you will be billed.) Of course, the hospitals make money off of all those epidurals and c-section births that they recommend to you as a client. If you follow the typical "doctor's orders" and do "whatever he says" you may end up with an epidural (at least) and maybe even a c-section birth because you have not informed yourself of anything else. Nancy tells us stories after stories of how women have been ill treated by hospitals and they were made to have a second c-section because they had a previous one, etc. The couples had no choices. There are couples out there who have doubted their need for a first c-section birth and if it was a real reason--they would like to have a chance at a vaginal birth after c-section - also called "VBAC". However, many hospitals won't allow it or doctors won't allow this. These couples have nowhere to turn. Many will go out of their way to find a midwife even in another state to have a VBAC home birth. It is possible. Some will have VBAC unassisted birth--as long as they have read up on what to do--why not? The doctor's won't let them try, but they desperately want that real birth experience. Getting cut is not a real birth experience--it is all fake. Nancy gives hope to the hopeless and tells couples how to do this and tells us about all kinds of interventions--did you know a hospital gown is an intervention? I never thought about it until reading this book.

The truth about C-Sections and how to prevent them

Because I have never had a hospital birth or been pressured to have a cesarean, the profoundly sad stories of my friends and family are what informed my opinions about c-sections prior to reading Silent Knife. I know far too many women who've ended up with unnecessary sections and subsequent infections and complications which robbed them of health and peace of mind and, in many cases, priceless bonding time with their infants. These women feel angry, violated, betrayed, and marginalized.
There are those who criticize Cohen and Estner for being over-zealous, exaggerating the problem, and portraying doctors in a largely negative light. The picture they paint is a realistic one, however. Unfortunately, the book is as relevant now as it was when it was first published in the 80's. At around 30% of U.S. deliveries, the c-section problem has gotten worse, not better.
Now that I'm involved in pregnancy and birth on a professional level (with Birth Works), I'm hearing more stories of women who think they've found liberal, supportive, natural birth-friendly OBs, only to have support for their birth plans yanked at the least sign of variation from the "norm" and intense pressure applied, when women are at their most vulnerable, to go for the c-section. In the end, whether or not you end up with a c-section may come down to how prepared you are to go to battle for your right and your baby's right to a natural birth.
Silent Knife is brimming with facts about the safety of VBAC versus repeat cesarean. It helps us understand statistics in a realistic context. I was astonished to learn that "uterine rupture" is a hugely misleading term. There are normal windows and separations of tissue--the result of healing, totally benign--that are wrongly included in statistics as ruptures. The image we get when we hear "uterine rupture", of a uterus literally bursting apart during a contraction, is a fear tactic anti-VBAC doctors use to bolster their arguments in favor of repeat cesareans. Statistics prove the dependability of uterine scars.
Silent Knife reminds us that birth is a sacred, not surgical, event. This book is crucial reading for any woman who wants to arm herself against the cut-happy establishment and demand evidence based care. It should be required for all childbirth educators and birth professionals.
